Hi doctor,

I am 30 years old and want to check if my T/E2 values are normal because I have been experiencing most of the symptoms of high estrogen levels for more than ten years, including an enlarged prostate, extreme fatigue, anxiety, muscle loss, belly fat, loss of libido, and knee and joint pains. About six months ago, I was checked for gastritis, and they said it was minor, with all readings, including thyroid, coming back normal. However, they recommended checking T/E2 and PSA levels since they noticed an enlarged prostate. Should I consider taking medication like Arimidex to inhibit E2 levels? My readings are testosterone 580 ng/dL, E2 43.6 pg/ml, prostate size 33 cc, weight 165.3 pounds, BMI 25.9, height 5'6", visceral fat 10.5, and body fat 27.9 %. Please help.

I read your query and understand your concerns.

First of all, I would like to inform you that your testosterone is entirely normal, but not estradiol. The maximum physiological level is 40 pg/ml, and the level above this rings an alarm bell, which should be investigated.

The increased level of PSA (prostate-specific antigen) along with prostate enlargement indicates there is abnormal aromatization of sex hormones, which could be due to simple overweight to sex steroid-secreting tumors. Regarding the treatment, there is a definite need for it, and Aromatase inhibitors could be the right choice for you. There are multiple options for this purpose, including Arimidex (Anastrozole).
