Hello doctor,

Does TRT reduce motile sperm count? Will hCG return to the pre-TRT level? My FSH and LH levels are very low, and my motile sperm count is zero. Previously it was 300 million pre-TRT. I have been taking 10 mg of Testosterone cypionate daily for 12 months. Although my testosterone levels have improved, sperm motility has reduced. I have undergone home-based fertility tests, blood tests, and hormone profiles. I have been trying for a baby. Please help.

I have read your query, and I can understand your concern.

I have read your reports (attachment removed to protect the patient’s identity). TRT (testosterone replacement therapy) taken for a long time may adversely affect sperm parameters. Your FSH (follicle-stimulating hormone) and LH (luteinizing hormone) are low. Was it low before starting TRT? If it was normal before starting TRT, then it is the effect of TRT. You can be benefited by taking injections of FSH 75 (Urofollitropin) IU twice a week and hCG 2000 (human chorionic gonadotropin) IU thrice a week. However, your other reports are normal except for lipids which may be again abnormal due to the side effects of TRT.
