Hi doctor,

I have a few bumps that start below my tonsils (especially on my left side) and then trace back into my throat. They are constantly present, even though they might be hurtful at times. They had been there long, and I wondered what they were. I have had tonsil stones for the past 12 years. And currently on medication.

I understand your concern. As I can tell from the image (attachment removed to protect the patient's identity), you are referring to the lumps on the posterior pharyngeal wall and below your tonsils. These are just lymphatic cells. They, together with tonsils, form part of what is known as the Waldeyers lymphatic ring (a ring of lymphoid tissue found in the throat). They have a defensive role. So, therefore, you do not have to be concerned because these cells protect your throat from infection. And the stones you mentioned are tonsilloliths (a calcified mass in the tonsils, which are the fleshy pads at the rear of the throat) and are common. So there is no reason to be concerned about them. But do you still have sore throats regularly, and do these stones hurt?

Thank you for the reply.

The one on the left gets sore and painful from time to time. The tonsilloliths had not caused me any discomfort since I learned to recognize them early and pop them out before they become severe. But because of my experience with the stones, I was anxious that something was wrong with the lymph nodes. They are not evident in images.

I understand your concern. Since you have a sore throat, it is most likely due to a tonsil infection (tonsillopharyngitis). I will prescribe some medications, and it would be preferable if you could begin them immediately. Tablet Amoxyclav (Amoxycillin 500mg and Clavulanic Acid 125mg) 625mg twice daily. Tablet Aceclofenac 100mg with Serratiopeptidase twice daily. Tablet Pantoprazole 40 mg once daily in the morning before food. And tablet Levocetirizine once daily at night. Along with it, do salt water gargling three times daily. In terms of whether you require a tonsillectomy procedure, how often do you experience severe throat soreness in a year? Do you experience any fever during such episodes?
