Hi doctor,

I have been feeling hot all over my body for the past few weeks with body weakness, diarrhea, muscle weakness, feeling cold, and chills. I was diagnosed with Salmonella typhi after a sample of my blood was taken and tested. I was given medications, but symptoms persist after finishing drugs for the past three days. At first, I was diagnosed with malaria and was treated by injections and medicines before I was diagnosed with typhoid. Please, let me know what medications I should take. In the past, I took Aquaclav, Flagyl, and Ibuprofen.

Your signs and symptoms suggest intestinal infection, which could be due to typhoid or viral infections such as COVID-19 or hepatitis A. But if your blood culture showed Salmonella typhi organisms, then it is the likely cause. It would have been better if you had shared the blood culture report to see organism sensitivity to antibiotics. According to the organism pattern in your region, I am prescribing the medication. Continue to take it for two weeks, and you will eventually be feeling good.

Typhoid, or Covid-19 or Hepatitis A.

Probable diagnosis

Gastroenteritis secondary to typhoid.

Treatment plan

Stop your current medications and start the one I am mentioning. 1. Capsule Ciprofloxacin 500 mg one tablet orally twice daily for 14 days. 2. Tablet Panadol (Acetaminophen) 500 mg per oral thrice daily as needed for fever 3. Tablet Famotidine 40 mg one tablet orally once every day before meals. 4. Tablet Zincate (Zinc Sulphate) OD 50 mg per oral once daily. 5. Tablet CaC (Calcium Lactate Gluconate, Calcium carbonate, Vitamin C, Vitamin D3, and Vitamin B6) 1000 one tablet orally once daily. 6. ORS (Oral rehydration solution) sachet, one sachet twice daily.

Preventive measures

1. Avoid spicy or oily food. 2. Drink 10 to 12 glasses of water.

Regarding follow up

Follow-up after two weeks.
