Hi doctor,

My mother is 48-year-old. She occasionally feels dizzy, and there is eyesight blurring and numbness in her jaw. Sometimes she finds ants in her urine. She has hypertension, and she is taking medication, tablet Amlodipine Besylate 10 mg. She has diabetes and is taking Metformin for two years. She got her BP results from the clinic, and it was 150/100 mm Hg. Yesterday she went to get her fasting blood sugar test result, 403 mg/dL. The reference range is from 74.0 to 105. What medicines should she take, and what diet should she follow?

Her fasting blood sugar over 400 mg/dL is very high. What is the HbA1c (glycated hemoglobin) value which will show your average sugar over the past two to three months? How much is she using Metformin? I suggest her the following advice. She can increase it to 2 gm in 24 hours if she is taking less than that, and if she is already on maximum dose, she needs to add another medication like Sitagliptin 50 mg once daily to start with and may increase it to 100 mg if still, sugar levels are high. Diabetes has many complications, so it is not just to bring down the numbers but to monitor and treat complications associated with diabetes. For example, check your eyes for retinopathy, kidneys for urine albumin creatinine ratio. Check your feet. Lifestyle modifications are the best treatment of diabetes. Low carbohydrate diet and no added sugar. No sugary juices. No bread, rice, pasta, and potatoes. If you cannot stop, at least take as little as possible. Take more vegetables, salads, and fibers. Fruits only as a sweet dish. Do not take mango grapes and bananas. Apple not more than one piece. Strawberries and other berries are fine, along with peach, apricot, and small amounts of plums. Intermittent Fasting will also help you to control your diabetes. You can fast for 16 hours, like taking ur dinner at 7 PM and not eat anything after that. Next, you can take breakfast at 11 AM. During fasting, drink water and green tea without sugar. You can extend fasting to 24 hours then 36 hours once your body adapts. You can do fasting two times a week, or the way you feel is good to control your sugar. Drink more water and sleep well. Exercise or at least walk 30 to 40 minutes daily. The more you will practice these things; the better will be your control. Check your sugar levels frequently. Your fasting target should be 90 to130 mg/dL, and two hours after eating should be 150 to160 mg/dL. HbA1c should be less than seven. These targets can vary in individual patients depending upon their diabetes duration and other relevant issues. Regards.
