Hello doctor,

My perineum stings when I urinate and is often itchy. I had seen blood spotting when I wiped my genital area after urinating. I am worried about this condition and not sure what to do. Kindly help me.

I understand your concern.

Considering your condition, I think it may be due to infection or laceration. I suggest you do the following tests if possible.

1. Urine routine and culture test.

2. Pap smear of vaginal discharge for microbiology and others.

If the tests mentioned above are normal, it might be due to an anal fissure. You need antibiotics, analgesics, and antihistamines along with douching of the vagina and anus with antiseptic solution (sitz bath) after wee or pee. Along with medications, I suggest you follow specific measures as mentioned below.

1. Avoid spicy foods and preserved tinned foods.

2. Avoid carbonated drinks.

3. Avoid coffee and tea.

4. Take a high fiber diet.

5. Apply analgesic ointment.

6. Take a mild laxative.
