Hello doctor,

I had an echo, stress test, and 14-day Holter tests, that came back showing arrhythmia or reduced heart function. I have brought it up to my regular doctor that sporadically I get ectopic beats when I go out for my evening walks. I have noted them on different days and found that my heart rate was 104 bpm during a 30-minute walk, 104 bpm whilst shoveling snow, 108 bpm while shoveling snow, 112 bpm with 2 bpm extra betas at the same time during cool-off from a 30 minutes walk, and 127 bpm during a 30 minutes walk. After each extra beat, I did not feel dizzy, chest pain, or anything but anxious. Why is this happening and is it a cause for concern? Please advise on what constitutes a double ectopic beat, I get an extra beat then 5 to 10 seconds later get another extra beat. Is that a double extra beat?

Your Holter monitoring report is normal. The palpitations you felt, were not due to heart rhythm issues. Few premature beats are normal. These increase due to anxiety, excessive caffeine intake, or electrolyte abnormalities. You said echocardiography was normal, so no need to worry. Are you taking any medicines? An increase in heart rate during exertion is normal due to an increase in demand for oxygen. Few PVCs are also normal occasionally three consecutive are also normal. PVCs (premature ventricular contractions) are of concern in the case of ischemic heart disease or another structural heart disease. Your echocardiography and Holter test are normal. If frequent and bothersome, you may take medicine for it which will slow down your heart rate a bit. Tablet Nebivolol (Lactose monohydrate, Microcrystalline cellulose, Crospovidone (type B), Crospovidone, Magnesium stearate) 5 mg once a day dose may be increased depending upon response. Consult your specialist before taking medications. Avoid anxiety, and do regular exercise. Kindly share your Holter report and echocardiography. Thank you.
