Hi doctor,

I am a 27-year-old male. I am circumcised and recently noticed irritation on my penis near the head. I have a small white dot just above the circumcision line. And I also have two to three pink spots surrounding my tip. Some papules seem to be slightly inflamed and irritated. There is no pain, itching, or discharge from any of these blemishes. I masturbate daily, and we have sexual intercourse frequently. My partner and I both have had blood work done, showing no STD’s. Do I see some fungal infection on my penis? Not sure why my penis would become infected after five years of faithful marriage life. Please help.

I have gone through your case. In my opinion, you are having a genital lichen planus. It has nothing to do with STD (sexually transmitted disease), and it is also not part of fungus infection. It is an inflammatory response. However, a further detailed examination is also required. My advice is to mix Clotrimazole and Hydrocortisone cream in equal amounts and apply two times a day. Clotrimazole is antifungal. In lichen planus, potent steroids are used. But it has been observed that in genital areas, if potent steroids are applied, then fungus can grow.

I will buy OTC brands of both recommended medications and mix equal half parts and apply twice daily. Is it recommended to use for a few days after the symptoms get cleared to prevent recurrence? Or should I stop the treatment once symptoms subside?

Please use it for atleast three weeks. But remember to take an opinion after three weeks before stopping the treatment. It sometimes takes prolong treatment, but the response varies from patient to patient.
