Hello doctor,

I am 40 years old, weighing 167 lbs and measuring 5.9 feet. I am feeling a discomfort in the abdomen for past one year. I did a blood test, and the result showed a high bilirubin total of 44.5 (normal range 0 - 21), ALT (alanine aminotransferase) or SGPT (serum glutamic pyruvic transaminase) of 46 (normal range 0 - 41), direct bilirubin of 10.1 (normal range 0 - 5). My hepatitis B and C test are negative. Liver function test and sonar report shows fatty liver.

I have seen your reports (attachments removed to protect the patient's identity), and I think you have a slight rise in total bilirubin and predominant indirect bilirubin, which is a common finding. We see such a condition in Gilbert syndrome, a genetically determined condition with problems in the transportation of the bilirubin from the liver. The liver function tests do not explain the pain since it is a benign condition and does not cause pain.

I wanted to know further details,

  1. Where precisely in the abdomen do you feel discomfort? For example, is it the upper abdomen above the belly button, around the belly button, or below the belly button?
  2. Is it severe enough to cause you to sit and stop working, or can you continue with this discomfort?
  3. Do you feel gas bloating or excessive wind?
  4. Do you notice the discomfort worse in a fasting or fed state?
  5. Is there anything that brings up the discomfort?
  6. Do you have a change in bowel habits?
  7. Does this discomfort go with anything?

You do not seem obese, so I do not think you have fatty liver. Let me know the answers to the above questions, and I will be able to solve your problem.
