Hello doctor,

I used a skincare product a year ago, and since then it has provoked a massive breakout. I used many creams and it subdued eventually, but now I have spots around my chin and on the highs of my cheeks that breaks out from time to time that leads to big white heads. Kindly help.

I read your query and understand your concern. I have gone through the pictures attached (the attachment was removed to protect the patient's identity). I could notice predominantly multiple closed comedones or whiteheads along with acne along the jawline, which is usually suggestive of hormonal imbalances. The severity would be between grades two to three and ideally, this would require oral Isotretinoin (Butylated Hydroxyanisole, Edetate disodium) to be started after necessary evaluation. One thing to note is closed comedones are the most resistant and toughest to treat acne form and usually take eight to 12 weeks to clear. As there is a noticeable amount of pimples along the jawline, you might require an evaluation for PCOD (polycystic ovarian disease) if there is a history of irregular menses. You can meanwhile apply Benzoyl peroxide 5% gel over the pimples daily evening for two hours and consult your dermatologist for starting Isotretinoin. I hope I have cleared all your doubts.
