I am 26 years old and have had acne problems for five years. My skin is a combination type. There are tiny bumps and whiteheads on both cheeks along with acne spots and scars. Please recommend a cream for acne scars, spots, and pimples.

I hope you are doing well. I have gone through the information that you have provided and viewed the image attached (attachments removed to protect the patient's identity) and understand your concern. Acne could be due to multiple reasons including hormonal fluctuations, any underlying medical conditions like PCOD (polycystic ovarian disease) which is quite common among young females and may also be accompanied by other symptoms which may or may not be present such as menstrual irregularities, unwanted facial hair growth, weight gain, hair loss on the scalp, etc. Other causes of acne include a family history of a similar condition, using any skin care products which trigger the formation of acne, dietary factors, climate factors, stress, any underlying medical conditions, or any medications which are being taken for the same.

My advice:

Consult your specialist doctor, talk to them, and take medications after their consent.

1. Ahaglow S (Salicylic Acid, Glycolic Acid, Aloe vera, and Citric Acid) foaming face wash. Apply a sufficient quantity of the foam on the face and massage in general circular motions for almost a minute and wash off. They need to be used twice daily in the morning and at night.

2. Peroclin 2.5 % gel (Benzoyl Peroxide and Clindamycin). This may be applied on the pimples alone in the morning after using the face wash. May continue for months together till there is a remission of acne.

3. Cleargel AP gel (Adapalene and Clindamycin) This may be used initially on alternate nights on the pimples only. This can be used for the first six weeks on alternate nights and then can be used for 12 to 18 weeks or till the acne completely fades off.

4. Capsule Zitcare (Zinc Methionine and vitamins) to be consumed at night after dinner for two months.

5. Capsule Microdox-LBX (Doxycycline and Lactobacillus) to be consumed after breakfast and after dinner (twice daily) for seven days. Make sure there is no history of allergy to any antibiotics before consuming this medication.

6. Tablet Pantop (Pantoprazole) 40 mg to be consumed 30 minutes before breakfast and 30 minutes before dinner for seven days to prevent acidity which can be caused due to antibiotics.

7. Make sure to avoid using any moisturizers or sunscreens.

8. Zitmoist gel (Sodium Hyaluronate and Allantoin gel) in case you experience dryness which could be a part of the medications. This gel can be used as a moisturizer to help with dryness.

9. Avoid oily and fried food, sweets, red meat, milk, and dairy products. Make sure to consume more fruits, vegetables, fiber, and at least 12 glasses of water per day

10. Chemical peeling treatments can also be considered which are great for improving your skin texture, reducing acne, reducing pore size, etc. This procedure is usually done at an aesthetic clinic by a dermatologist. In this procedure solution is applied on the face and kept for around five to ten minutes and then cleaned up. It is usually done every two weeks for at least six to nine sessions. This is a very inexpensive and superficial procedure without any side effects and can be considered for improving your skin texture, whiteheads, oil, and sebum secretion.

Acne as mentioned can occur due to multiple reasons mentioned above and can be on and off on exposure to any of the above factors. Make sure to avoid any possible factors which can trigger the formation of acne.
