Hello doctor,

I have suffered from AS for 10 years. I manage it by diet, walking, and a few simple exercises. But for the last three months, I have been suffering from right leg pain with neck pain, right eye redness and pain (HLA-B27 positive), and getting some pain relief tablets along with B12 injections, and Vitamin D3 60K tablets. I have been diabetic for 10 years.

Please suggest better medicines.

Ankylosing spondylitis is a chronic problem that is lifelong. However, treatment is very important because the fusion of joints may cause problems with the movements of the body. Redness of the eye is called scleritis and can occur in AS. NSAIDs (non-steroidal anti-inflammatory drugs) are the first line of drugs, and if the patient fails to improve with them, then biologics are given. You need to get evaluated and examined by a rheumatologist for disease activity to look at which treatment you will need at this point. The activity is calculated by physically examining the patient and doing a few investigations, such as ESR (erythrocyte sedimentation rate) and CRP (C-reactive protein).
