Hello doctor,
I am a 26-year-old. I am working in a bank for the last year. I am having back pain on the right side thoracic region while sleeping on my back or sitting for the last six months. I have antral gastritis. I have done an MRI of the thorax which came normal. Ultrasonography of the whole abdomen is also normal. X-rays of the chest, and blood tests like LFT are also normal. My doctor said that it might be postural pain, but it is like a crawling pain just like an insect crawling under the skin sometimes. I have taken physiotherapy which has given some relief, but still, there is a pain in the scapular region on the right side. Currently, I am taking Nucoxia MR, Caltrol, and Pan 40. Is any other test required? I am attaching my scan reports for your reference. Please guide.

Hello doctor,
It is mainly below the scapular region at one or two points and like radiating towards scapular region. It is a sharp pain, sometimes crawling and creeping pain, sometimes pain radiates to front right abdomen. In physiotherapy, I have taken ultrasound, physio TENS and IRR for seven days. As I am working in bank, I do not find much time for exercise. Does it signify any organ related problem? But, my USG of abdomen is normal. While sitting and bending in front there is a sharp crawling like feeling in one of the two parts below right scapular blade. Please suggest what should I do? I have attached all my physio reports and clinical prescriptions for your reference. Please help.

I have read your clinical prescriptions and the reports attached with it (attachment removed to protect patient identity). You seem to have postural back ache with myofascial trigger points. You have to improve your sitting or working posture. Include yoga or back muscle strengthening exercises in your daily routine. From your attached clinical notes, there has been almost every form of physical therapy being tried but no report regarding any relief (temporary or partial) with any of it. You can take a ball and ask some other person from family to massage the painful areas with ball in circular motion with pressure.You can do it yourself using a single sock with ball in it and hanging it over your shoulder. You require medication in the form of muscle relaxant for a few days along with physical therapy mentioned above. Keep me posted about the results.
