Hello doctor,

I have had muscle twitching everywhere in my body for the past seven months. I had low levels of vitamin D and taking supplements for three months. The twitching has gotten better for sure. However, I have had hand and feet cramps and stiffness. Now freaking out thinking I have got atrophy in my right hand, but I do not struggle to do any daily activities. My right thumb also shakes when I hold it in a specific position but not at rest. Please help.

I would like to know some details about your symptoms.

What is your age? Since when you started to have muscle twitches? Is it for seven months only or before also? In which body parts, you have these twitches? Please mention the body locations. Are these twitches visible to the eyes or only felt inside? Have you noticed in which situations these twitches are more or get triggered?

Do you have any visible thinning of muscles or weakness of hand grip in hand or leg? Do you have impairment of sensation in any body part, neck or lower back pain, pain in elbow joints, or other joint pains? If yes, in which joints? Are you taking some medications (apart from vitamin D supplements)? If yes, please mention names and dosage? Do you have any significant weight loss in recent time? Since when are you noticing right thumb shaking and atrophy in the right hand? I would like to know if this atrophy visible by eyes or you have some weakness in doing activities by thumb or both?

Have you consulted a neurologist and got a neurological examination? If yes, please attach their findings. Have you gone through some investigations for your symptoms? If yes, please send relevant reports. I would suggest you make a small video of your right hand's shaking and close pictures of thumb areas, wherever you feel atrophy, and send it here as an attachment.

How is your sleep routine? Do you get sleepy soon after lying in bed? Or it takes a long time? Once got sleepy, do you have frequent awakenings during sleep? Do you feel fresh on awakening from sleep in the morning? How is your usual mood in day-to-day activities? Happy toward the sad side (thinking about past events), or the worried side (thinking about future things, irritable, or something else)? How is your appetite?

Investigations to be done

T3, T4, and TSH (thyroid-stimulating hormone). Serum vitamin B12. NCV (nerve conduction velocity) and EMG (electromyography) of all four limbs (including brachial plexus study). MRI brain and cervical spine with screening whole spine.

Regarding follow up

Please revert with the above-mentioned information for better understanding and further assessment. Also, please send pictures of atrophy areas and video of thumb shaking and if have a neurological assessment paper or any investigations reports.

I have had the twitches for only seven months. They are visible to the eyes. I get them in my left arm and occasionally on my legs. I do not have a loss of grip strength or any weakness in general. I have had three car accidents in the past two years, which has given me neck and back pain. No pain in larger joints and no weight loss. I have noticed the thumb shaking for a couple of months now. I do not know if I have atrophy or not. It looks like my right hand may be smaller than the left. I have considered getting tested by a neurologist but did not get yet.

You have not given a reply to all my questions nor sent any attachment. I am still not even knowing your age. As you mention that you are not sure if it is atrophy, it is really difficult to conclude. The only thing which you affirmed are twitchings over body parts that are visible.

I suggest you consult a neurologist physically for a detailed clinical examination. Also, you may need to undergo some investigations. I am mentioning those, but you should discuss them with your neurologist about them.

Investigations to be done

Fasciculations.

Treatment plan

As per your description, these muscle twitchings can be fasciculations, the cause of which need to be evaluated. Fasciculations can be due to multiple reasons (hyperthyroidism, anxiety, benign fasciculation syndrome, fasciculation cramp syndrome, motor neuron disease). You need to discuss in detail with your neurologist with a physical neurological examination.
