Hello doctor,

I have a spot on my penis shaft. I took an STD test, and it showed negative. However, I am concerned that it could be something more serious. The genitourinary specialist prescribed me Eumovate. I have no other symptoms. What do you think it could be? Should I be concerned? Kindly help.

I understand your concern. The problem is that you did not mention the period between the appearance of the lesion and the STD (sexually transmitted diseases) test. For example, syphilis, which could cause such lesions in stage 1 and has a diagnostic window of about 10 to 12 weeks. If the lesion was there already (before three months) and you tested now, the test would be valid. Shorter intervals make the result invalid. That is the case with many STDs (human immunodeficiency virus - 10 to 16 weeks, hepatitis C virus - three to six months, and syphilis - 10 to 12 weeks). So, I suggest you repeat the standard syphilis test - Treponema pallidum hemagglutination (TPHA) when you are sure that 12 weeks have passed. If it is still negative, then a tiny biopsy probe (which does not hurt) can be taken to check what is wrong with the skin. Visual diagnoses of the skin are extremely unreliable. Therefore, I would always suggest a small diagnostic biopsy. Kindly revert in case of further queries. Thank you.

The probable cause is STD (sexually transmitted disease), which is still possible.

Investigations to be done

Do Treponema pallidum hemagglutination (TPHA) again, minimum of 12 weeks since the lesion appeared. If negative, a small diagnostic biopsy can be done to get a diagnosis from a histopathologist.

The differential diagnosis are syphilis, wart, eczema and others.

Probable diagnosis

The probable diagnosis is telomere biology disorders (TBD).
