Hello doctor,
I have noticed some black spots or a black pimple for a while near my penis. Wonder what is that? I have attached the images.

Kindly send a more clear picture delineating the suspected "pimple" lesion (attachments removed to protect the patient's identity). Please answer the questions below for a better understanding of the condition.
1) Did you observe any change in the growth of the lesion?
2) Did you notice any pus formation on the pimple-like lesion?
3) Did you notice redness, swelling, or itching around the lesion?
4) Did you have any systemic symptoms like fever or other symptoms that may help correlate with systemic disease?
5) Did you have any unprotected sexual exposure?
6) Did you have any history of chickenpox or herpes in childhood?
The probable cause behind this could be herpes or any benign lesion, and the lesion looks probably like a penile nevus with herpes.
Do not engage in sexual intercourse before getting the results.
Get the following investigations done:
1) STD panel tests- Herpes, Chlamydia, Gonorrhea, Treponema pallidum, Hepatitis B & C, HCV, HIV.
2) General - CBC (complete blood count), ESR (erythrocyte sedimentation rate).

Hello doctor,
The last time I had intercourse was two years back, which was protective; then, I did my STD test after a year, and it came out negative since then; I have never had intercourse. I noticed these black moles or pimples. I do not know what it is called, but I have had it for quite a long time, definitely more than a year or two or could be three years not sure what the exact time is, but I have been seeing it for a while. It does not itch, burn, or pain and has no swelling or redness. I have no fever and no history of chickenpox or herpes.

In my opinion, these are the following possibilities in differential diagnosis:
1) Pimples and blackheads- Most pimples and blackheads are harmless and often resolve in a few months with basic at-home care, such as good hygiene and warm compresses. A mild infection or blocked pores may have caused it, often observed as dark spots on the scrotum.
2) Hyperpigmentation- It occurs when some areas of the skin develop more pigment. This can appear as a variety of rare skin lesions, such as freckles, moles, sunspots, and stretch marks, and are harmless.
3) Angiokeratoma- This condition causes small blood vessels to widen, resulting in non-cancerous skin lesions that may be dark red or blue.
Please share all your patient records with any skin specialist or dermatologist.
