Hello doctor,

I am a female nearing 40. My partner and I are sexually very active. A year ago, I was diagnosed with PID, and the pap smear revealed nothing else. We both got treated and were fine for a few months. I am very clean; I often wash before and after sex. We are both into oral sex as well. For the past few days, I have been experiencing excessive discharge (with no smell or pus), which is almost white. I also have a burning sensation at the tip of my left labia and pain in the lower abdomen. There is no burning sensation when I urinate. The burning sensation occurs after we have sex. I do not use perfumed soaps, but I do use a feminine wash. I am currently taking thyroid medication - Thyronorm 100 mcg. Recent laboratory tests, including a Pap smear and ultrasound, were conducted during the days when I had PID. Is this PID again, or could it be something else? My partner is fine, and we use protection during this time. Can you suggest some medication? What could be causing these symptoms?

Based on the symptoms you described, it appears you must be having yeast infection like candidiasis. I would suggest you take a tablet Fluconazole 150 mg orally, single tablet each for you and your husband. Clingen forte (a combination of Clindamycin, Tinidazole and Clotrimazole) vaginal pessary to be kept vaginally every night for six nights. If there is still no improvement, you can consult your gynecologist for examination and do pap smear and ultrasound if abdominal pain persists.

Thank you for the reply.

I have a problem again. The previous one got resolved. I am sexually very active with a single partner and am very conscious of my personal hygiene. However, for the past few days, I have an itchy sensation each time I become wet or excited. This has never happened before, and I also noticed that one of my labia is slightly enlarged. This is concerning to me, and I sometimes see a discharge, although there is no smell whatsoever. I do not use harsh soaps and always use recommended intimate washes. We only use virgin coconut oil as a lubricant during our encounters.

I believe it could be vulvovaginal candidiasis. I suggest you use Candid B (a combination of Beclomethasone and Clotrimazole) cream locally twice daily for a week. If symptoms do not resolve or worsen, it is better to pay a visit to your gynecologist. Get well soon.
