Hi doctor,

I am 24 years old. I have left chest tightness and some discomfort for the past twenty days. I had done ECG (electrocardiogram), echo (echocardiogram), and TMT (treadmill test). All the tests are normal. But this discomfort happens every time continuously. I have a dull ache in the back, no extra pain with physical activity. The problem mainly occurs when sitting down for work. I have vitamin D deficiency.

It is good to know your cardiac workup is fine. As you have vitamin D deficiency, take vitamin D supplements. Tell me about the pain. Is there any history of trauma or heavy load-bearing? What makes pain decrease, and what makes it go worse? Does it occur with rest? Does light to moderate exercise or physiotherapy helps? Take muscle relaxants such as Zanaflex (Tizanidine Hydrochloride) and Norflex (Orphenadrine Citrate) thrice a day. Have a fasting lipid profile done if you have a family history of a cardiac issue, or your lifestyle is sedentary. If pain worsens with rest, jog or stretch every two hours for a few minutes.

I mentioned that pain stays for the whole day, with or without exertion. When I sleep, I do not feel any pain. There is no heart disease in my family, and I do not exercise at all.

Do regular exercise and meditation. Physiotherapy may be needed if the pain is persistent and bothersome. Simple painkillers like Paracetamol can be used. Ibuprofen topical cream can also be applied. Muscle relaxant (Orphenadrine Citrate) can be used thrice a day for three days. Massaging with olive oil also helps. Do not worry about this pain as it is not cardiac.
