Hello doctor,

We have been trying for a baby for the past four years, but we did not get any fruitful results even after taking many medications. With all hormone tests and with normal reports (complete blood count, TSH, LSH, random blood sugar, HSG, laparoscopy, and biopsy), she is still not able to conceive. I am taking Ovacare Forte and Thyronorm 25 mcg. Can you please guide us?

There are often many reasons why a couple cannot conceive. I did not see a semen analysis in your query or laboratory results (attachment removed to protect patient identity). If this has not been done, that would be the next step. If this has been done and is normal, other likely causes could be the mild endometriosis noted on the attached operative report, even though it is not causing any distortion of the pelvic anatomy. Most couples will try intrauterine insemination (IUI) if sperm count is normal, as this is usually the less expensive option. If IUI fails to achieve pregnancy, IVF (in vitro fertilization) is typically the next step. There is also a test to check for ovarian reserve or if quality oocytes are present called AMH or anti-Mullerian hormone. I am uncertain if you and your partner have undergone any of these therapies or tests that I have mentioned, but based on the information given here, these would be reasonable options to pursue.

Thanks for the reply.

We have undergone all the tests, and our gynecologist says that my wife's egg quality, uterus line everything is good. It has been monitored during Clomid cycles as well without any injectables or medicine. Her egg growth was good, and we have undergone IUI twice, but still, we could not succeed. It is because of stress and anxiety my wife is struggling to conceive, and she has been depressed a lot. Please give us hope that we can have our baby naturally, as we are not ready for IVF right now. Can my wife conceive naturally, as she is totally fed up going to the hospital, and that is building pressure on her? Please let us know, do we have a chance of conceiving naturally.

Yes, of course, there is always a chance of conceiving naturally. Stress and anxiety are very common when going through something as difficult as infertility. You and your wife are not alone, and finding support from other couples going through the same thing may be very helpful. I have often experienced that when a couple realizes that they have done everything they can do and decides to let things happen as they may conception often happens. I have seen couples begin adoption proceedings because they decided they were not going to have stress over trying to conceive and take a different path. You have done all of the right things, and you and your wife can find hope and ways to reduce stress and anxiety. IVF is a big step, and you most definitely need to be in the right place for that, but it can serve as a comfort to know it is there as an option.

Thank you for your valuable suggestions.

I think at this moment we need certain hopes to lead a life that gets us out of stress and depression. Slightly edematous on the right tube according to the reports, and it is not a problem for my wife. I believe both the tubes were patent, as it is mentioned in the report, but a slight edematous medical statement worried her a lot. Please let us know about that edematous statement. I really want to take my wife out of this depressed zone, so the answer from you about this will help us a lot.

Yes, both tubes were open when I reviewed the operative note (attachments are removed to protect the patient's identity). However, edematous tubes could possibly have fluid in them, which could impede fertilization. This would be something to consider if you decide to pursue IVF. I am not a fertility specialist, but many specialists in your country will actually remove the tubes before IVF to prevent the fluid in the tubes from causing problems.

Thank you. Finally, I just want to request you to please suggest to us what we should make changes in our lifestyle to get a fruitful result. As of now, we have made changes in food habits taking nutritious food and doing exercise walking every day. Apart from that, suggest tips to relieve this stress and how to cope with a positive attitude. At this point in time, we really need your suggestion to motivate and see the world with good hopes. Right now, ICF, we are not preferring that is our last option to conceive naturally. Please suggest us some changes.

It sounds like you have already made many positive changes in your lives, and that is a step in the right direction. Getting exercise, ideally, 30 minutes of moderate activity five days per week, is what is recommended. Eliminating processed foods is also very important, along with ensuring lots of colorful fruits and vegetables. Your wife should continue to take a prenatal multivitamin or folic acid 400 micrograms daily. In my experience, when things are not happening when you want them to, it is often for a reason. Many times that reason may not be apparent, but when it is the right time, things will work out for the best in the long run.
