Hello doctor,
The woman has been using IUD for five years. She was in a relationship with her boyfriend. She has taken an ultrasound exam and I am attaching all 15 ultrasound images. I need to know if the woman is pregnant or not. She claims she has been pregnant for the last four months. However, she has not been sexually active for the last four months. Can she be pregnant?

There is no gestational sac seen in these images (attachment removed to protect the patient's identity), which means according to this there is no pregnancy. According to her dates, she should be around 20 to 22 weeks of gestation, and this week's baby could be clearly seen in USG (ultrasound) images. Even with Copper-T pregnancy may occur, because no contraceptive is 100 % sure. There is a failure rate of around 0.2 to 2 %. She may be having amenorrhea (absence of menstruation) but there could be some other reasons for that. For more confirmation, she can also check beta hCG (human chorionic gonadotropin) levels. Thank you so much for your response. The woman claims doctors refused her to take any beta hCG level tests twice, and she refuses to take any urine tests as well. Thank you for your expert opinion on this, I am going to conclude this as non-pregnant.

Beta hCG levels can be easily checked with a simple blood test as we routinely do. There should not be any reason to refuse. If she is claiming she is pregnant for the last four months then clinically also it is easy to confirm. Her belly should be swelled up to or just below the umbilicus and the fetal heart can be audible easily with Doppler.
