Hi doctor,

After my second dose of COVID vaccination, I started experiencing different sensations in my body. The second day after the vaccine, I started getting intense feelings in my body. First, I would get extremely dizzy, then my speech would almost become slurred and confused, then my eye lids would become super heavy, and I felt weak and tired like I was going to pass out. This happened several times, and I thought I was having a stroke. I went to the emergency room, and it was not a stroke. Sometimes migraines would follow. I was told they found an arachnoid cyst, but they said it might be an incidental finding and nothing to worry about. I got my booster, and everything is happening again. I was supposed to see a neurologist, but he decided to take a vacation which pushed my appointment later, after two months. So I am trying not to freak out, and I am trying to figure out what to do. I am just worried something is happening, and we could be preventing it or taking action.

I am under Hydroxyzine for sleep, zinc tablets, vitamin D and, vitamin K supplements. I performed CBC test and CT scan.

Please help me with the current situation.

1) How long do these symptoms last?

2) Did you ever experience palpitation or breathing difficulties or did you experience any feeling of sinking or any fearfulness?

3) How many times in a week do you experience these symptoms?

4) How is your sleep routine? Do you fall asleep soon after lying in bed? Or does it take a long time? Once falling asleep, do you wake up often in between sleep? Do you feel fresh after waking up in the morning?

5) How is your usual mood in day-to-day activities?

6) How is your appetite?

Hi doctor,

Thank you for your concern. I have answered your questions below.

The symptoms usually last for about 10 minutes. But then, sometimes, I feel tightness in my chest. I feel dizzy, almost like the same feeling as being drunk. Then my words and speech seem off. I am sometimes confused too. Then, I get this tired feeling in my body and feel like I will pass out. And I get tingly and goosebumps throughout my body. Such episodes occur two times a week. I have trouble sleeping a lot, and I rarely wake up rested. I have been checked for sleep apnea, but I do not have it. Generally, I am optimistic. I believe life is a gift, and there are good opportunities and beauty all around. But then, when this happens, I think maybe I am sick or that I might have a disease. My appetite is generally pretty good. I have a hard time losing weight even though I eat good food.

By your description, I think it to be an anxiety attack or a panic attack.

But cardiac problems need to be ruled out before labeling it.

I would suggest you for some investigations pertaining to the heart and if nothing comes out, then to consult a psychiatrist for detailed psychological analysis and further management.

Investigations to be done

ECG (electrocardiography), 2D Echocardiography, Holter monitoring.

Differential diagnosis

Anxiety attack, Panic attack.

Treatment plan

Till you can meet a psychiatrist, I suggest you try the following measures: 1) Daily jogging 20 to 30 minutes in the morning. 2) Avoid alcohol or smoking. 3) Maintain regularity in sleep and wakeup timings
