Hello doctor,

My husband is 51 years old and had bout of confusion this morning which he never had before. He kept repeating the same questions, I would answer, and a minute later, he would ask the same questions. He had a bad sleep a couple of nights, and we had sex this morning. Could this be a low blood pressure issue? He otherwise feels fine. No other symptoms. After several minutes of talking through what day it is and what happened yesterday, we finally got to where he seemed to be less foggy. He has a family history of heart issues and overweight, but has never experienced any overt heart-related problems. He takes Xyzal and vitamins, but that is about it. Please help.

I can understand your concern. According to your statement, your husband has been suffering from confusion, repeating the same type of questions, even getting the answers to those questions, lack of sleep for a few days, and difficulty in remembering.

Your husband may suffer from delirium. In the case of delirium, such type of clinical symptoms may be found. Sudden delirium may result from some underlying health problems like urinary tract infections, acute retention of urine, TIA or mini-stroke, stroke, medications, low blood sugar, head injury, lungs or heart diseases, carbon monoxide poisoning, alcohol poisoning, or withdrawal, etc.

Again your husband is currently using Xyzal, which contains Levocetirizine dihydrochloride may increase the risk of delirium symptoms. Even low blood pressure has been reported in studies to trigger brain damage and cognitive impairment and aggravate the delirium symptoms.

If your husband's recent blood is low, try to raise his blood pressure by taking oral saline fluid. Again, if the Xyzal is the culprit to develop such clinical features, then stop taking Xyzal may help to subside his present sufferings over time.

Some investigations may help diagnose the exact causes for his present sufferings if he does not get rid of his present sufferings by taking above mentioned measures.
