Hello,

Welcome to icliniq.com.

Thank you for your query. Your sugars are slowly creeping up. I suggest you need change to Davi 10 or 100 once daily. It contains Vildagliptin, which you are already taking with Dapagliflozin. Diabetes has many complications so it is not just to bring down the numbers but to monitor and treat complications associated with diabetes. Check your eyes for retinopathy and kidneys with urine albumin creatinine ratio. Check your feet, lifestyle modifications are the best treatment for diabetes.

1. Low carbohydrates diet and no added sugar.

2. No sugary juices. No bread rice pasta or potatoes. If you cannot stop at least take as less as possible.

3. More vegetables salads and fiber. Fruits only as a sweet dish. Do not take mango grapes and bananas. Apple not more than one piece. Strawberries and other berries are ok along with peach apricot and small amounts of plums.

Intermittent fasting will also help you to control your diabetes, you can fast for 16 hours like taking your dinner at 7 pm and do not eat anything after that next you can take breakfast at 11 am. During fasting just drink water and green tea without sugar. You can extend fasting to 24 hours and then 36 hours as your body adapts. You can do fasting two times a week or as you feel comfortable it will control your sugar. Drink more water sleep well, and exercise at least walk 30 to 40 minutes daily The more you will practice these things the better will be your control. Check your sugar frequently. Your fasting target should be 90 to 130 two hours after eating 150 to 160 HBA1c should be less than 7. These targets can vary in individual patients depending upon their diabetes duration and other relevant issues. So you can first try lifestyle modification as I mentioned above for two weeks and if your sugars are coming down in the target range continue with the same medication but if it is still high you can change your current medication to Davi 10 or 100 once daily as I mentioned above. Let me know after one month.

Thank you.

Patient's Query

Hello doctor,

I will act accordingly. My feet are burning sometimes. Please advise.

Answered by Dr. Zulfiqar Ahmed

Hello,

Welcome back to icliniq.com.

It is most probably due to diabetes as well which affects your nerves of the feet causing diabetic neuropathy. If it is more troublesome I suggest you can start Pregabalin 75 mg once at night for one week then 150 mg once at night for one month. Once your symptoms improve you can decrease it to 75 again and then take on alternate days and then stop it.

Thank you.

Same symptoms don't mean you have the same problem. Consult a doctor now!
