Hi doctor,

I am having pain while passing urine after having intercourse. The pain continues for two to three days. Earlier I had similar pain for the last three months. I have consulted the local urology doctor, and I am taking medicine to treat bladder issues first (Vesicare 5 mg). If this does not settle doctor said he would advise treating the prostate issue. Please advise.

I would be happy to give you my opinion. The most common problem in males with such symptoms will be in the prostate and bladder. Proper examination of prostate and semen culture can diagnose prostatitis, which is chronic in your case. After semen culture, we can select the sensitive antibiotic and can be used for four to six weeks. Some other medications like alpha-blockers and anticholinergics can be added to the antibiotics to improve the symptoms. Bladder cystitis can be a second diagnosis and can be confirmed by cystoscopy with bladder biopsy. In my opinion, since your urologist has started treatment for bladder problems, you can carry on with it and observe for improvement. If there is no improvement, then you can do a semen culture to confirm prostatitis. Please note that chronic prostatitis is of four types. Two types are bacterial infections, and the other two types are without bacteria, so if semen culture is negative, we will still go with antibiotics in combination with alpha-blockers and anticholinergics.
