Is Medroxyprogesterone related to my bleeding after masturbating?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I was diagnosed and surgically treated for endometriosis a year and a half ago. Since then, I have been on the Depot Medroxyprogesterone Acetate shot, and I get it every eight weeks. When I do not get the shot precisely every eight weeks, I continue bleeding and having pain.

For the past few months, I have been bleeding after masturbation. However, there is no penetration, so that is not the reason. But it does not make sense for me to even bleed in the first place because I am getting the shot more frequently than recommended. I am currently on a year-long exchange program abroad, and the bleeding and pain from my endometriosis are getting worse. But before I left, I contacted my Gynaecologist, and she told me to wait till I got back, and we would run some hormonal tests.

Is there anything to worry about? I am just worried because my symptoms have worsened, and I am not near my doctor.

I am currently on tablet Sertraline (Sertraline Hydrochloride) 100 mg, tablet Carbergoline, and Depo-Provera (Medroxyprogesterone Acetate) shot.

Kindly help.

Thank you.

Hi,

Welcome back to icliniq.com.

I can understand your concern.

It would be better if you could share your complete medical history. Since many drugs are taken together, side effects are seen.

Injection of Depo-Provera (Medroxyprogesterone Acetate) has the primary side effect of erratic bleeding. Also, endometriosis can cause painful menstruation or pain during intercourse. Therefore, it is better to stop Depoprovera immediately and start combined oral contraceptive pills for three cycles (consult with a specialist doctor, discuss with him or her and take medications with their consent).

Take a high vaginal swab, complete blood count, thyroid function, cervical smear tests, and a transvaginal scan. Please send me the reports to discuss further.

I suggest you take (consult with a specialist doctor, discuss with him or her and take medications with their consent) tablet Tranexamic acid 500 mg 8 hourly and tablet Ponstan (Mefenamic Acid) 500 mg for three to five days. In addition, include iron supplements and iron-rich foods like apples and carrots.

1) Get complete treatment for any health issue you have.

2) Follow strict hygiene.

3) Wear soft and comfortable undergarments.

4) Use condoms during intercourse to avoid transmission of sexually transmitted infections.

5) Involve in recreational activities and quit recreational drugs.

6) Follow a balanced diet and maintain a healthy weight and body mass index (BMI).

I hope things will be better.

Regards.
