Hi doctor,

I have a problem with erectile dysfunction. It is a problem relating to how I ejaculate. I can only ejaculate from lying face down and humping into my hands or the bed. It is a condition called 'traumatic masturbatory syndrome.' For some reason, I can not ejaculate with an erection. I can get an erection, but I can never finish. I liked some advice on why this is and what I can do about it, and if it could be a urological problem. I have already tried Cialis and Viagra, but it does not work. I have also started low-dose testosterone replacement therapy recently. I do not take any other medicines other than asthma inhalers. Please suggest a solution?

This could be a primary or secondary issue (newly developed). If it is primary, then the cause will be unknown, and management will be more difficult, but if it is newly developed, this is the functional issue due to compression of lumbar vertebral one and two. As that is a place on the spinal cord for ejaculation reflex and any interference with this level vertebrae can cause delayed ejaculation or anejaculation. I would suggest taking an MRI of the lumbar spine to check further.
