Hi doctor,

I am a 39-year-old male. I am experiencing premature ejaculation and erectile dysfunction. My problem is, in the first attempt, I ejaculate early, and when I try to ejaculate a second time, I cannot erect my penis firmly, so I fail to penetrate. It has been happening for the last few months, but now I am very worried about it. My medical history is absolutely fine till last month. In the last three weeks, I have experienced tachycardia twice. I consulted a cardiologist. My blood pressure was 140/90 mm Hg before five days. I have done stress tests, ECG, 2D echo, thyroid, and blood sugar levels. All reports are normal. But doctor suggested tablet Concor AM 2.5 mg. I am stressed that I am failing to satisfy my partner. Please give me good treatment for sexual wellness so that I can live stress-free. I will attach my recent laboratory reports.

I have gone through your reports (attachments removed to protect the patient's identity). Your ECG (electrocardiography) result is satisfactory—no worries about that. However, you should see a hematologist because the discrepancy between low hemoglobin and a high red platelet count is slight but striking, especially when we take MCH (mean corpuscular hemoglobin), MCV (mean corpuscular volume), and other values also into consideration.

I fully understand that you want a fast treatment. And yes, there is one, a combination of a very specific SRI (serotonin reuptake inhibitor) drug and good old Viagra (Sildenafil). However, both can also mess with your complete blood count (CBC), and I want first to be sure that these anomalies are just caused by something harmless like dehydration.

So, please see a hematologist and let him find the reason for these anomalies. There are multiple reasons for these changes, from dehydration to severe illness.
