Hi doctor,

My wife had a gallbladder attack last week. It was triggered by ice cream, dark chocolate chips, and syrup. The pain was mild at night and was triggered by ground beef. The attack was severe and required ER visit for pain relief. The pain resolved after taking rowachol, chanca piedra, Ibuprofen, and Oxycodone. The doctor performed an ultrasound and diagnosed multiple gallstones. The doctor said that the duct was not blocked and immediate surgery was not required. We began immediate treatment via dietary changes and a natural regimen based on online studies (rowachol, chanca piedra, TUDCA, Lecithin, Turmeric, Milk Thistle). We would like to add Ursodiol to our regimen, but our local nurse practitioner said that she is not aware of medical treatment for gallstone dissolution and recommended surgery. Kindly help.

I understand your concern. I suggest the following for managing gallstones:

1. If gallstones are asymptomatic, immediate surgery is not required. It requires lifestyle modifications like a low-fat diet and weight reduction.

2. If gall stones are symptomatic, but the patient is not a surgical candidate due to co-morbidities or other reasons, supportive treatment like intravenous fluids and antibiotics are given to reduce the inflammation. But, this has a risk of gall bladder rupture or further complications.

There are a few radiological procedures like ultrasound-guided drain or tube placement within the gall bladder. These procedures are performed for very sick patients who are not fit for surgery. Definitive surgery is performed after the condition of the patient improves. There is no medical treatment for dissolving the gallstones without surgery, and those methods that are not there in the clinical guidelines are not recommended. I hope this helps you.
