Hello doctor,

I have sudden onset of sharp, gnawing belly pain radiating through to my middle back. The pain is constant whether sitting, standing, or laying down. My gallbladder was removed two years back. I also have a bloated belly and it is very tender just below the spleen. Apart from these I also have acid reflux and I vomited twice. I have no relief from over-the-counter medications. What should I do? Please help.

I understand your concern.

It seems like an acidity and reflux disease. It may take a little longer time to respond. I suggest you do the following -

  1. Avoid a fatty, oily, and highly spicy diet.

  2. Go for a walk after having food instead of taking a rest.

  3. Have multiple small meals instead of heavy meals.

  4. Have regular sleep habits.

  5. Avoid stress.

  6. Add lots of green leafy vegetables, and fruits to the diet.

  7. Avoid smoking and alcohol if any.

  8. Elevate the head end of the bed during sleep and rest.

I suggest you take tablet Pan DSR ( Pantoprazole plus domperidone) 40 mg or tablet Rabpill LS (Levosulpiride and Rabeprazol) before breakfast once a day for two weeks. Along with this, you should have syrup Sucral-O (Sucralfate and Oxetacaine) or Gelusil (Aluminium hydroxide, Magnesium hydroxide, and Dimethicone) two teaspoons three times a day for a week. For pain, I suggest you take an antispasmodic tablet Dicyclomine 20 mg, or a tablet Hyoscyamine 10 mg whenever needed, up to three times a day. If these are not helping, you should visit the emergency department once for intravenous medicines and rule out other causes.
