Hi doctor,

I am not feeling good for more than four weeks. I do not have an appetite and so I have lost some weight. I have night sweats. My whole body is weak and uncomfortable for the whole day. It is getting worse day by day. I have a burning sensation in my nose and throat. I have more coughing after eating. I also have a burning sensation in my stomach. It gets worse after eating. It seems to be acid reflux after eating. I have extreme pain in my calf and I have difficulty in moving it. I feel very tired with severe night sweats. I wake up every half an hour every day at night. Please suggest.

Regarding your symptoms, you can have a differential diagnosis of GERD (gastroesophageal reflux disease). It is basically reflux of acidic content of the stomach into food pipe or esophagus. To confirm it, I suggest you consult a gastroenterologist after the pandemic settles down and you need to do an endoscopy to confirm the diagnosis. Try to avoid drinking alcohol, citrus juice, tomato-based products, and coffee. After taking meals, have a walk and wait for two to three hours and then lie down. Keep a pillow to keep your head elevated while sleeping. Stop taking Omeprazole and start taking tablet Esmoprazole. Take syrup Sucralfate one tablespoon before 30 minutes of taking Esmoprazole.

Investigations to be done

You need to do an endoscopy and urea breath test.

Treatment plan

You can take tablet Esomeprazole with Domperidone and syrup Sucralfate.
