Hello doctor,

For the past ten days, I have been suffering from the flu and a headache. I took Cefixime 200 mg and a combination tablet of Chlorpheniramine maleate, Paracetamol, and Phenylephrine. However, nothing seems to be working. After taking the pills, the flu and headache disappeared temporarily for just one day, but they returned immediately. My wife is also experiencing the same problem.

  1. You mentioned having a headache. Is it more like facial pain or a browache?
  2. Do you experience any headaches when bending down?
  3. Do you get temporary relief after taking tablets or from steam inhalation?
  4. Are you experiencing nasal congestion or a runny nose?

If you answer 'yes' to these questions, it is likely that you have sinusitis.

Additionally, I need the following details to provide a better treatment plan:

  2. Are you using nasal drops like Xylometazoline 0.1 % or Oxymetazoline hydrochloride 0.05 % to relieve nasal congestion?

If you have already tried all of the above suggestions and still have not found relief, it could be due to an allergy. Allergies can cause symptoms such as sneezing, nasal congestion, itching, and a runny nose. In that case, I would recommend using Mometasone furoate or Fluticasone furoate nasal sprays. Montelukast may also be prescribed to help reduce allergic inflammation in the nose.

To determine if the cause is an allergy, a few investigations are necessary. We need to perform an absolute eosinophil count and a serum IgE (Immunoglobulin E) test. To check for a bacterial infection, a diagnostic nasal endoscopy should be conducted. This test will provide information about the pathology inside the nose and help us understand why the infection is not healing. Finally, a CT (computed tomography) scan of the paranasal sinuses (PNS) should be performed. This will show the ostiomeatal complex, sinus drainage pathways, and any possible abnormalities.

Here are some general measures you should follow to help alleviate your headache and flu:

  1. Avoid smoking and drinking until you feel better.
  2. Drink three to four liters of hot water daily.
  3. Refrain from consuming chilled foods or drinks.
  4. Take frequent steam inhalations.
  5. Avoid direct exposure to the sun, and stay away from dust, traffic, smoke, and pollution.
  6. Consume healthy, nutritious food and consider taking vitamin supplements.
