Hi doctor,

I recently had a very dark stool with some black bits, a lot of mucus when I pass stool, and only passing large amounts of mucus with no stool and having an enlarged swollen rectum. I also have visible blood when I wipe. I first went to my PCP about having constant constipation and having dark stools with mucus, and she told me it was IBS and prescribed me an Enema and a Linzess. When those did not help, the mucus became more prominent, the stool color was darker, and being able to see blood after wiping. I reached back out to her. She gave me another laxative to take daily. She said to wait a month and go back if it did not help, and then she would perform a rectal examination. I only have two to three bowel movements a month. My stool is usually soft, with ragged edges and floats, and I always have bloating. I was hoping to get a second opinion.

Chronic constipation leads to the stagnant of stools, and thus, there is bacterial action, dryness, and other chemical changes that may contribute to the dark color of your stool. Due to bleeding from an upper gastrointestinal problem, the stool color would be dark black and tarry inconsistency and would be very foul-smelling what we call in medical terms as melanic stools. The appearance of your stool is not melanic. Do not worry.

But I feel you have hemorrhoids or piles that are engorged vessels near the anal verge, so you get blood on wiping.

Kindly answer a few more questions to help you more.

  • Do you see blood in drops or sprays after passing stool?
  • Did you notice blood mixed with the stool?
  • Do you have any weight loss?
  • Do you drink at least 2.5 liters of water daily?

Hi doctor,

Thank you for your reply.

Sometimes I will see drops on blood after roasting stools. I am not positive if it is mixed with the stool. There is some red tint to the water, but besides that, the stool itself has been either dark-colored or sometimes has black specks or black balls in it. I had some weight fluctuation, and I have lost about 8 lbs over the past month. I do drink that much or more than that in a day.

I think there is nothing to suggest that you have any of the alarm signs warranting urgent evaluation. However, you may require a rectal examination as advised by your PCP (primary care physician), which will help confirm whether there are piles. You may as well request him for a rectal examination at an earlier date so that he can prescribe some medicines, including laxatives for the same. On the whole, do not worry, relax, and drink plenty of water and take enough fiber.
