Hello doctor,

I am a 70 year old. I have issues with high BP and diabetes. The doctor has given me a medicine for the heart and cholesterol. Are these good medicines? Is there any prevention I need to take? In some reports, it says that something is passing through the urine. I do not understand clearly but, the doctor has not given any medicine or preventive action for this. Can you suggest something from your end?

I have gone through the reports (attachment removed to protect patient identity). Regarding your urine doubt, I did not find anything in the urine report and there is nothing abnormal mentioned regarding urine. So, please see to it and reupload the report if available. Secondly, your blood sugars, blood pressure, and ECG show abnormalities. You need to be on medicine for it. Your cholesterol is also slightly elevated. Being a diabetic and hypertensive, you are at a risk for coronary artery disease. Also, your ECG (electrocardiogram) is showing some changes. So, the next thing to do is, you should undergo an echocardiography and if abnormal, then you will need angiography and if normal, then you need stress thallium or CT coronary angiography. So, you need to be evaluated for it. Importantly, your BP and sugar should be controlled. So, if you have bought those medicine, please upload the photos of the strips or the name of the tablet. You should have a healthy lifestyle. Avoid fatty, oily and high calorie foods. Have a low-salt diet. Monitor blood pressure regularly thrice a day for one week, then once or twice a week. If BP is persistently more than 140/90 mmHg, then the medicine needs to be hiked up. Do regular exercises like brisk walking, jogging according to your capacity for at least 30 minutes a day and five days a week. Have lots of green leafy vegetables, fruits, and fish once or twice a week, and avoid meat. Avoid smoking and alcohol if any. There should not be abdominal fat deposition or obesity.
