Patient's Query
Hello doctor,
I had sex with a sex worker 42 days ago. We used a condom, but it broke during intercourse. Since that day, I have been feeling very anxious. I am a psychiatric patient and have been taking medication for anxiety and stress for a long time. I have attached my test reports. Please review the attached reports and provide your feedback.
Am I completely risk-free from HIV?
The first test was performed 25 days after exposure, and the second 37 days after.
Please help.
Thank you.
Hello,
Welcome to icliniq.com.
I have gone through your query and understand your concern.
HIV infection (human immunodeficiency virus) is a virus that attacks the body’s immune system. It usually has a window period of one to three months. During this time, test results can be false negatives. Although your current reports (attachment removed to protect the patient’s identity) are adverse, which is reassuring, we advise you to retest at 12 weeks to confirm the results.
Additionally, please get tested for HBsAg (hepatitis B surface antigen), VDRL (venereal disease research laboratory test), and anti-HCV (antibodies against the hepatitis C virus).
I hope I have answered your question.
Let me know if I can assist you further.
Thank you.
Patient's Query
Hi doctor,
Thank you for the reply.
I have attached reports that were taken on the third day after the sexual exposure. I have also done an HIV test for the girl. I am a high-anxiety patient, and waiting for a further six weeks is not possible for me, as I am living in constant tension and fear of HIV.
Is there any advanced technology test available at this stage so that I can be completely free from the fear of HIV?
I have been feeling cold for the past 15 days. This symptom was present at the fourth-generation test, conducted 37 days after the initial test. For the past two days, I have also been experiencing pain in my hip joints. Does this change your opinion in any way?
Please advise me and help me come out of this anxiety and fear of HIV.
Thank you.
Hello,
Welcome back to icliniq.com.
I read your query and can understand your concern.
It is good to hear that the baseline tests have been done. However, most viruses have a window period, and results can only be confirmed at the end of these periods. HBsAg (hepatitis B surface antigen) and anti-HCV (antibodies against hepatitis C virus) tests need to be repeated after 4 weeks. For VDRL (venereal disease research laboratory test), the test should be repeated after twelve weeks to confirm the results.
In the case of HIV, the fourth-generation test is considered highly accurate at the end of six weeks, but the World Health Organization recommends retesting at twelve weeks. However, it is unlikely that your results will change, so there is no need to panic further.
I hope I have answered your question.

Patient's Query
Hi doctor,
Thank you for the reply.
I have done the tests again, 45 days after the last exposure. Please review my attached reports and advise me, as I am living with high stress and fear of HIV.
Can negative results at this stage turn positive later?
Can I have unprotected sex with my wife?
Due to heavy stress, fear, and anxiety, I am unable to concentrate on my personal and professional life. Are these reports reliable within the six-week window period from the last exposure?
Please help.
Thank you.
Hello,
Welcome back to icliniq.com.
I read your query and can understand your concern.
I have gone through the reports (attachment removed to protect patient identity). Hepatitis B and C tests are negative, and there is no need to repeat them. Regarding HIV, the result is considered 98 percent accurate. The chance of a change in the report is negligible. You do not need to panic further.
I hope I have answered your question.
Let me know if I can assist you further.
Thank you.
Patient's Query
Hi doctor,
Thank you for the reply.
I am attaching the HIV RNA PCR test report. Please explain the interpretation of pathogen not detected.
What does it mean?
If I want to undergo HIV testing every 15 days until three months after exposure, which test should I do?
Can I have unprotected sex with my partner?
Please help.
Thank you.
Hello,
Welcome back to icliniq.com.
I read your query and can understand your concern.
Pathogens are infectious agents. If the report mentions pathogens not detected, it indicates that no virus was found, and the test result is negative. We do not advise testing every 15 days, as it is not useful. You can go for a single test at the end of 12 weeks to confirm the result.
If you still wish to continue testing, choose the ELISA (enzyme-linked immunosorbent assay) method. It is advisable to use a contraceptive method until the test result is confirmed. Try to use a condom for 12 weeks, as this would be ideal and safe.
I hope I have answered your question.

Patient's Query
Hi doctor,
Thank you for the reply.
My mental state is not in a position to wait until 12 weeks from exposure due to high anxiety, fear, and stress. Please suggest any advanced technology test that can provide a conclusive result within 52 days, eight weeks, or two months following the last exposure. Because of this stress and fear of HIV, my professional and personal life is heavily disturbed, and if I receive a clear conclusion, I will be able to relax.
I have also been experiencing a cold and cough for the past 20 days, which has added to my stress and fear. I am worried about my family, especially my wife and my child, and want to keep them safe. This is a humble request to suggest a high- or advanced-level test at 52 days, 8 weeks, or 2 months after exposure so that I can be completely relieved of stress.
Please help.
Thank you.
Hello,
Welcome back to icliniq.com.
I read your query and can understand your concern.
The most effective tests for detecting HIV at an early stage are fourth-generation HIV tests, which screen for both p24 antigen (a viral protein) and HIV antibodies. These tests are 98 to 99 percent accurate by the end of six weeks from the last exposure. This is the only test that can give better early results.
However, as part of the routine protocol, we advise repeating the test at the end of 12 weeks, even if the initial result is negative. You have already done the fourth-generation HIV test, and the result is negative. You do not need to panic, as the test was done well beyond four to six weeks.
The chances of your result changing are almost negligible. Try to calm yourself and settle your mind. You have not harmed your family. We understand your anxiety, but you must try mind diversion methods and start moving ahead.
I hope I have answered your question.
Let me know if I can assist you further.
Thank you.
Patient's Query
Hi doctor,
Thank you for the reply.
I have been experiencing a cold and cough for the past 15 to 20 days. I have taken medicines prescribed by a general practitioner and also from a medical store, but I am still not getting relief from the cold and cough.
Could you please prescribe suitable medicine for this condition?
Due to this persistent cough, I am also living in tension, as I am worried that this could be a symptom of HIV.
Please help.
Thank you.

Patient's Query
Hi doctor,
Thank you for the reply.
I have taken medicine prescribed by a general practitioner and also used a regular cough syrup from a medical store for about two to three days. I have been taking cough syrup for the last two days, but I am still not getting relief from the cough. I am only experiencing a cough, not cold symptoms.
Please help.
Thank you.
Hello,
Welcome back to icliniq.com.
I read your query and can understand your concern.
Your complaints could be due to pharyngitis (inflammation of the pharynx) or low-grade bronchitis (inflammation of the bronchial tubes). This usually takes two to three weeks to subside.
You can follow the advice of your general physician in this regard.

Patient's Query
Hi doctor,
Thank you for the reply.
The day before, I visited a massage parlor. At the parlor, I did not have any direct oral, vaginal, or anal sex with a commercial sex worker, but I engaged in the following activities with her:
I touched her breasts and pressed them for five to ten minutes.
I touched and fingered her buttocks for five to ten minutes by putting my hand inside her pants.
I touched her backside and fingered it for five to ten minutes.
I kissed her on the face, not on the lips, and she also kissed me on my face, not on the lips.
I was wearing underwear, and she was wearing pants. I had an erection, and semen was present inside my underwear. Semen came out of her underwear and touched her clothing.
As it was a commercial massage parlor, the bed was dirty and might have had dried semen drops from other people.
I noticed a pimple on my nose after coming home, and after taking a bath, a small amount of blood came out of it.
I have had a cough and a cold for the last two months. About 1 hour after the above activity, I had a coughing spell.
Please tell me if I am at risk of HIV. If there is a risk, how much is it?
Also, please tell me the shortest window period for HIV testing, as I am a high-anxiety patient and cannot wait for a long time.
Please help.
Thank you.
Hello,
Welcome back to icliniq.com.
I read your query and can understand your concern.
There would be no risk in the scenario you have described. There was no exchange of body fluids, so this cannot be considered an exposure to bloodborne pathogens. There is no need for any HIV testing.
A fourth-generation HIV test is considered accurate by the end of four weeks from the last exposure.
I hope I have answered your question.
Let me know if I can assist you further.
Thank you.
Patient's Query
Hi doctor,
Thank you for the reply.
To reduce my high anxiety, I would like to undergo testing. Please provide a complete description of the viral load (RNA PCR) test.
I also forgot to mention that I noticed a tiny cut on one of my fingers, which touched her breasts, buttocks, and other body parts. The cut was tiny, and I am not sure whether it can even be considered a cut, as there was no bleeding at any point. It may have just been superficial skin removal. Additionally, I hugged her for five to ten minutes.
Please help.
Thank you.
Hello,
Welcome back to icliniq.com.
I read your query and can understand your concern.
In such a case, you can go for a fourth-generation HIV test. This includes testing for HIV-1 and HIV-2 antibodies, as well as the p24 antigen assay. This test is highly accurate by the end of four weeks.
I hope I have answered your question.
Let me know if I can assist you further.
Thank you.
