Hi doctor,

I am a 50-year-old female taking bioidentical hormones for the past six months due to hot flashes and night sweats for the past six years, and it worsened last year. I also take NP Thyroid 60 mg, Testosterone, Estrogen, vitamin D3 tablets, multivitamins, MethylAssist tablets, zinc tablets, Quercetin, Curcumin, and Primrose supplements, and use progesterone cream. My blood test shows high B12 of 116.5 ng/dL, high testosterone of 240 ng/dL, low TSH of 4 mlU/mL, and low vitamin D of 10.3 ng/mL. Now I have hyperthyroidism with irregular menstruation, like once every three to five months. I did not have periods for the past three months. I have attached my reports.

From the reports attached (attachment removed to protect the patient's identity), you have hyperthyroidism. So I suggest you tablet NP Thyroid 45 mg once daily. Repeat the thyroid function test after six weeeks. I suggest tablet Vitamin D 1000 IU once daily, 4 hours after taking tablet thyroid NP 45 mg.

What about the high testosterone levels and low estrogen? I use combination creams in the evenings. Please help.

Due to the age factor, there is a low estrogen level. Replacement therapy, like bioidentical hormones, results in high testosterone levels and low TSH (thyroid-stimulating hormone) levels. Consult your doctor for alternative medications.
