Patient's Query

My wife is experiencing weakness and dizziness. Her face is trembling, her right hand is shaking and she's experiencing some confusion. Her temperature is normal however. She's also been experiencing neck and back pain since yesterday. She is 31 years old. Her family has a history of diabetes, but she herself does not.

Dear is there any history of vomiting? Any history of exposure to sun for exclusive time?

Possible causes accordingly to your history

1. Heat stroke or heat exhaustion.

2. Cervical spondylosis with compressive neuropthy.

3. Acute febrile illness with generalised weakness.

4. Hypotension with generalised weakness.

5. Electrolyte imbalance.

6. Hypoglycemia

7. TIA.

8. Coronary artery disease etc.

But until examination is done it is difficult to say what it is. But give her rest, you can give coconut milk for drink. Check blood pressure and get it done ECG. If blood pressure is low then you may give ors solution also orally. If symptoms not improved please get her to physician he will examine and treat her accordingly. Take care.

Hi Dr. Thanks for your reply. No exposure to sun or heat for a long time. She is in out of station currently, I am stuck in home town because of Covid. She does not have a fever or a temperature. What is TIA?

TIA is transient ischemic stroke means (one day stroke). In this patient may complain weakness in particular limb and recover with in 24 hours. If the neck and back pain acute means possibility of viral myalgia may be she will get fever with in 24 hours. So give Paracetamol 650mg thrice a day. If she is under confusion state means you have to take her to physician.

Oh I see. Additional symptoms: She feels dizzy only when walking or standing up. When lying down she does not feel it.

That means possibility of Benign positional vertigo or postural hypotension (hypotension on standing position). so check blood pressure once. Give tablet Vertin 8mg thrice a day may useful. If no improvement you may use 16 mg thrice a day. Thank you.

What to do besides taking a vertigo tablet? Any food items, exercise, lifestyle changes to take?

Dear this condition is most common. This may present like acute condition only. It may aggravated by taking cool drinks or freeze water. Also patients with pharyngitis or sinusitis or middle ear infections or allergic rhinitis or sedative drugs may suffer more. So do water vapour inhalation thrice a day and salt water gargling thrice a day for five days. Avoid cool drinks. Avoid allergic food that aggravate the condition. Epleys manover is the good exercise for this. You may search in net you will get this exercise. Take care.
