Hello doctor,

I have been taking tablets Bupropion 150 mg and Sertraline 50 mg for the past month. I occasionally I take Agomelatine 25 mg or 50 mg, depending on how bad my insomnia is, to help with the initial insomniac phase of Bupropion. I plan to increase the dosage of tablet Bupropion to 300 mg in the next few days and perhaps Sertraline too. I am also considering adding tablet Lamotrigine to balance my mood swings. How much is a safe dose? Initially, I took the lowest dose for three days, but it made me suicidal and gave me unsettling dreams, so I dropped it. Still, I figured it might be good to try this medicine as my psychiatrist suggested I should take a mood stabilizer. Once my sleep pattern improves, I am planning on dropping the Agomelatine. I want your opinion on the dosage of each of these drugs.

I can understand your concern. You have BPD (borderline personality disorder). There is non-pharmacological management for it. I suggest you to follow few instructions. Exercise regularly for 30 to 45 minutes. Start meditation for about 20 minutes. Eat healthy and home-cooked food. Drink enough water. Spend quality time with friends and family. Spend time on creative hobbies like reading good books, writing journals, painting, drawing, and learning new skills or language or musical instruments. Take enough 7 to 8 hours of sleep. Avoid tobacco or alcohol, or any substance abuse. Also, you can increase the dose of tablet Bupropion (antidepressant) up to 450 mg and tablet Sertraline (antidepressant) up to 200 mg if you are not stable. It will help you to deal with your depressive features. If you have no depression, just stay above your routine dose and add low-dose tablet Lamotrigine (anticonvulsant). You can add low-dose tablet Lamotrigine as mood stabilizer if you want. You can start with 100 mg daily. Kindly consult a specialist, talk to them and take medications with their consent.
