Hello doctor,

After the last consultation, I took the prescription recommended by you. I also had the thyroid test, and it was all fine. I was able to have a good sleep. And my efficiency of working increased a bit.

But now, again, I am having intrusive thoughts, and there is always something going on in my mind throughout the day, even while I am sleeping. And it is very difficult to focus. Can I continue the last prescription as it is? And also, there is an unusual smile while communicating with others, which creates many difficulties. I cannot stop it. I tried it very hard. I am attaching the prescription file. I do meditation in the morning. Does it has any negative effect, and should I stop it?

The prescription given was,

Tablet Fludac - 20 mg.

Tablet Oleanz - 2.5 mg.

I understand your concern and the problems encountered by you while discontinuing the medication. At the same time, I am happy to know that you are doing well in many domains with little medication and no problematic side effects other than a possible unusual smile.

As I explained during our first consultation, you need to continue medication for a long time which may last six months and more. If you are having proper relief while taking medication, you can continue the same medication for another month. If you have these intrusive and attention problem thoughts despite taking medication, we have to increase the dose of fluoxetine to 40 mg, which should be taken at the same time as in the past.

Please be reminded that you are not supposed to skip even a single dose as the proper medication is of utmost importance for the complete treatment of your condition.

Regarding your unusual smile, if that is the same thing you described during your first consultation, then there is no problem. I hope with continuing medication, that will get alright. If the unusual smile is a new thing, then probably the side effects of these medicines or something that needs to be investigated. Suppose you do not feel any improvement feel free to contact me.

It is almost very difficult to modify medication with a small piece of information that you described above.
