I need advice on the week five ultrasound for IVF pregnancy. hCG has been doubling appropriately.

The scan (attachment removed to protect patient identity) shows a single intrauterine gestational sac of about four to five weeks. However, the sac is a bit irregular. It could indicate early failure. A follow-up with transvaginal ultrasound is recommended at one-week's intervals. The yolk sac and fetal pole would be visualized in the follow-up scans.

Could the angle have been bad? I am obese. These were the only photos I had. My hCG was probably 5000. Also, this was an IVF.

It is not about a bad angle, but the sac looks irregular. The best we can do is wait for a week's time and repeat the scan, this time with a transvaginal probe and we would be clear to see a fetal pole and possible heartbeat in it too. And if the sac is not growing as per the period of amenorrhea, then it is definitely a failure.
