Hello doctor,

I have had small pimples filled with white pus. These tend to develop into bigger keloids, which are hard and sometimes itchy. These are now making me feel very uncomfortable, and I would like any treatment to get rid of them. Some are small, some are large, as in the pictures. Please help. I have been applying scar gel for two weeks.

It is a keloidal scar due to the overgrowth of granulation tissue, collagen at the healed skin injury site. Keloids are an excessive fibroblastic response to mechanical and or chemical injury. Genetic predisposition/keloidal tendency to develop keloid may be contributory. These are red raised plaques. They have a sharp, often irregular claw-like projection, which may continue to enlarge peripherally on the body's vulnerable parts, including the upper arm, chest, upper back, and neck. Use Retinoic acid in the strength of 0.05 to 0.1 percent gel or cream or solution may be applied two times a day for two to three months. Also, 0.1 to 0.5 of intralesional steroid of Triamcinolone or Hydrocortisone acetate (25 mg/ml) is administered either alone or in combination with Hyaluronidase every week. The intralesional injection may be repeated after one week to facilitate quick recovery. Since when they appeared? Do you have a surgical history? Is there a history of the use of Contratubex gel? Contratubex (Heparin sodium and Allantoin gel) is a scar-specific treatment for new and old scars. It is applied on the clean scar and a little away from the center to the edges or along the length of the scar for approximately two minutes rubbing gently. Compression plaster may improve its efficacy. The application is required to be made two times a day. The treatment is to be continued for a period of three to six months. Duration depends on the nature of the scar, whether old or new. Since when scar appeared? Is it an old or new scar?
