Hello doctor,

I am a 74 year old female with a height of 5.4 feet, and weighs around 112 pounds. There is a slight pain near the left side of the neck (that prompted us to go for the complete blood test). There are no other symptoms and I am not on any medication. I attached the blood test reports. The lipid profile shows various values out of range: Total cholesterol - 252 mg/dL, triglycerides - 193 mg/dL, LDL - 169.4 mg/dL, VLDL -38.7 mg/dL, CHOL / HDL - 4.71 mg/dL, LDL / HDL - 3.17 mg/dL, and non-HDL cholesterol - 198 mg/dL). Please suggest if any medication or consultations are required. Appreciate it if you could also let us know what needs to be done for the serum creatinine levels. Thank you.

I read your query and understand your concern.

A low serum creatinine with other tests being normal means a low muscle mass. You could do some exercises. The LDL (low-density lipoprotein) cholesterol is elevated and will need intervention. In addition to aerobic exercises, you should take a small dose of Rosuvastatin 10 mg after consulting your specialist doctor because of low serum creatinine and low muscle mass. It is better to taper off Rosuvastatin over three months and rely on lifestyle modifications like aerobic exercise, swimming, and avoiding fried foods. Take boiled vegetables, and oily fish (in curry form), avoid deep fried foods, and coconut oil. Thank you for choosing icliniq. If I understand correctly, since my mother is 74 years old, will walking be good enough exercise, or should she do something more strenuous?

Understand that Statin will have an impact on low serum creatinine, so it is better to taper off within three months. I read it is good to have proteins to raise the serum creatinine levels, and then having red meat will have an impact on cholesterol.

Can you please suggest other ways to increase protein diet that will not impact cholesterol?

Fish and white of an egg are good protein sources. And do not cause a deleterious effect on cholesterol. So are Bengal gram and nuts.

Hi doctor,

Per your advice, my mother has been taking Rosuvastatin 10 mg for the last three months. Now, her other blood report is attached. The Cholesterol levels have all come down to normal. Total Cholesterol 147 mg/dl, HDL Cholesterol Direct 44.6 mg/dl, LDL Cholesterol -Direct 88.7 mg/dl, and triglycerides 136 mg/dl. So now can she start to reduce Rosuvastatin to less than 10 mg or stop altogether as I see low serum creatinine?

She is 74 years old. She is now regularly going for her long walk.

Rather than abruptly stopping Rosuvastatin. she can take 5 mg on alternate days. That should suffice.

Unfortunately, there is no data on the stoppage of statins. Of course, we can give them a drug holiday and see after a month. But overall, I feel she should be able to take Rosuvastatin on alternate days.
