Hello doctor,

I get low heart rates of 40 to 45 bpm while sitting or lying down, and when I get up and walk, it will return to 60 and above. I also went for a long walk for about an hour, and my heart rate was between 78 to 90 bpm. I have had this issue for more than eight years. My query is, do I need a pacemaker now? Thanks.

Thanks for reaching out to us. I have gone through the result of your investigations (attachments removed to protect the patient’s identity). Your ECHO (echocardiogram) and ECG (electrocardiogram) are within normal limits and do not show any abnormalities. It is normal for some people to have a heart rate below the normal range. Heart rate usually changes a lot depending on activity. In some athletes, for example, it can get as low as 30 beats per minute. It does not mean that your heart is diseased. You do not need a pacemaker. Do you have symptoms like fainting or lightheadedness? Regards.

We generally advise pacemakers in conditions like hypertrophic cardiomyopathy, neuro-cardiogenic syncope, etc. We can also think of a pacemaker if the heart rate is below 40 bpm with symptoms. Otherwise, there is no need for intervention. I advise you to stop checking your pulse frequently as it will worsen your anxiety. Best regards.
