Hello doctor,

My partner and I had unprotected intercourse. Her periods were due 10 days back which she unfortunately missed and the pregnancy test came positive just two days back. We have decided to abort and she has taken Mifepristone by 10.30 am. I would like to know when is she supposed to take the Misoprostol. The name of the kit is Unwanted Kit. Please advice.

I have gone through your history and understand your concerns.

Abortion pills can only be taken under the supervision of a gynecologist because they may cause severe bleeding, infection and retained products of conception. We usually give Misoprostol after 48 hours of taking Mifepristone. Consult your specialist doctor, discuss with him or her and with their consent take the medicines.

She has already taken Mifepristone today morning as I said earlier. I want to know will there be any changes seen or felt after taking the Mifepristone? or the changes can be felt only after taking the Misoprostol? Please advise.

Yes, I know she took but it will only cause detachment from the wall of the uterus. So, bleeding occurs only after the Misoprostol.

The most important doubt is that she seems to be very normal in her activity without any pain. Is it normal? We are a bit scared and worried whether the first dose worked or not. Then, can I ask her to take the Misoprostol after 24 hours as mentioned in the guide that came along with the kit? Or should she wait for 48 hours as mentioned by you? Also is it possible to know whether the abortion process is completed after the course without going to the doctor or the clinic? This is to prevent an embarrassing moment.

Yes, it is normal as it does not cause any symptoms in some patients and some patients may feel pain in the abdomen. I always prefer to give the first dose on the first day and then Misoprostol on the third day morning and in the evening the second dose. However, consult your specialist doctor, discuss with him or her and with their consent take the medicines. She will feel extreme pain and products expulsion after taking the Misoprostol. You can go for a pelvic ultrasound in six to seven days after completion of the medicines.

As we discussed earlier, yesterday my partner had taken the first set of Misoprostol. Today by 10.40 am, she started to bleed within three hours of taking it. She is having pain as you said. Can she take any painkiller? If so, please suggest some. She has planned to take another set of two tablets by 4.30 p.m. Is there anything that I should advise her to watch for?

If the pain is bearable then she should not take any painkillers because it may decrease the expulsion process.She should see for bleeding and keep watching the products that come out. Take healthy and light food.
