Hi doctor,

I was diagnosed to have porokeratosis through a biopsy. I used Imiquimod cream and it was gone. After three years, the area started itching again now. Is it due to the recurrence of the same disease?

Which part of your body was involved? What type of porokeratosis (keratinization disorder) it was? Please send image.

Like in case of actinic porokeratosis when exposed to sunlight or radiation, there are chances of recurrence. When associated with an immunosuppressive disease, there can be appearance of new lesions. You should strictly avoid sun exposure and use sunscreen. Imiquimod cream or 5-Fluorouracil cream can be used.

Thank you so much for your quick reply.

It has been in my genital area for around 14 years but was diagnosed after around 12 years. I also have a history of testicular cancer. There was a white spot initially; however, there is no scar right now after applying Imiquimod. Therefore, the image will not help. It was genital porokeratosis and I want to know if it can regenerate or recur. I have also undergone cryotherapy in that area.

First of all, porokeratosis of the genital area is rare. There are very few case reports. Few studies have mentioned no association with malignancy. Some case reports have mentioned no history of recurrence at the same site after the excision of the lesion. But, porokeratosis can recur. Without any image, it is very difficult to comment if you have a recurrence of porokeratosis. If you notice any new lesions in the future, then please revert back with an image.

I am attaching the image here. There used to be a scar, which is no longer in that area. Please let me know any medicine to prevent recurrence. The area is presently a bit hard and itchy. Kindly prescribe a medicine.

At present, no active management is required. There is no recurrence of lesion at present (attachment removed to protect patient identity). If there is intense itching, then you can apply DesOwen cream (Desonide) at night for 15 days. As it is a mild steroid, do not use it for longer duration.
