Hi doctor,

I am a 57-year-old woman and recently started to have a lot of hair fall. Usually, when I comb or wash my hair, a lot of hair fall happens. But lately, the amount of hair I am losing due to hair wash is worrying me. It seems like I am losing the hair from the root. I underwent surgery to remove my uterus a few years back due to fibroids. I do not take any medications now. Is this a symptom of something, or can I change my diet or use medicine to control the hair fall? Kindly help.

I understand your concern. It would have been better if you attached a few clinical pictures of your scalp. The most common causes of hair loss in females are pattern hair loss and telogen effluvium. Sudden diffuse hair loss primarily indicates telogen effluvium, which usually starts within a few weeks to months of any infection, illness, surgery, or major trauma and stress. Kindly answer me the following to help you better. Is there any history of illness, infection, or surgery in the last few months? Do you have a history of fatigue, weakness, or thyroid disorders? Is the hair fall diffuse, or is the thinning more along the middle? Have you done any blood tests recently like hemoglobin or thyroid? If yes, please share them. When did you get your uterus removed exactly? Please revert with these answers, so I can provide you a probable diagnosis and suggest appropriate treatment. Hello doctor,

Thanks for the reply. I was COVID positive four months ago. I am fully recovered now. I do not have any illness or infection at the moment and have not undergone any surgery in the last few months. I do have fatigue sometimes. But, I do not have it often, and it is happening for a long time. The hair fall is diffuse, it is not happening from the middle. However, I can see fewer hairs in the front of my head. No blood tests or thyroid tests done recently. It has been more than eight years since my uterus was removed. I have attached a picture of my scalp. Thank you.

Thanks for sharing the details and the picture (attachments removed to protect the patient’s identity). Right now, it seems that you have telogen effluvium and a component of FPHL (female pattern hair loss). In FPHL, thinning is along the middle and frontal areas. And in telogen effluvium, there is a sudden increase in hair shedding after any illness. In your case, it might be COVID-19 (Coronavirus Disease-2019). I suggest you get your hemoglobin and thyroid checked once and start following medications as mentioned below. Take Tablet Keraglo Eva (Gamma-Linolenic acid and multivitamins) once daily. Apply Tugain (Minoxidil) 2% lotion 1 ml twice a day on the scalp at the root of the hair. Use Scalpe (Ketoconazole and Zinc Pyrithione) 2% shampoo twice per week. Revert after six weeks. Take care.
