Hello doctor,

I am suffering from skin infection mainly on my groin, buttock, waist, leg, and hand. The doctors are saying it is ringworm. Now I am under medication for the last two months. The tablets I am taking are Itraconazole 100 mg twice a day, Terbinafine 250 twice a day, Grisovin FP 250 two times a day, and Lulibet cream. It is responding to medicines but suddenly with a day, it appears again with red spots which are too itchy. I have done my blood test also and do not show any sugar. Please suggest.

Your reports are normal (attachment removed to protect patient identity). Never take all antifungal medicines at one time as they damage the liver and you will develop resistance to medications. Stop everything now. Start with capsule Syntran 100 mg (Itraconazole) twice daily for three weeks, Lulibet cream for local application, tablet Teczine 5 mg twice daily for three weeks, and KZ soap (Ketoconazole, Aloe vera, etc.). Do not apply or take anything other than this for three weeks. Review after three weeks.

As per your advice, I have been taking medicines for the last 21 days and during these days it has not reappeared again. I can say 75 percent improvement. But still, big patches are seen on my buttocks, groin, and loin area. I have attached a photograph of the same. As per advice, I am taking Itra 100 and Levocetirizine 5 mg twice a day, and Kz soap. Please also let me know any diet control I have to follow during medicines. e.g. non-veg, alcohol, and fermented food.

Do not stop continue for another three weeks same medications. No diet restrictions and avoid alcohol during taking these medications. Avoid sweating or else fungal infection can occur again and again Bath twice daily and use antifungal powder like Flucos dusting powder to keep the area dry.

Currently, I am applying Lullicanazole only at night and in the morning bathing with KZ soap. Is it advisable to apply Lullicanazole two times even after bathing with KZ soap? Also will these black patches subside?

Depending on the severity you can apply up to twice daily. Instead, you can apply Luliconazole at night and apply powder during day times morning and afternoon. Black patches will subside with time. Apply coconut oil as well.

I stay in a cool city so there is no sweating here. Also sweating in the groin area is understandable but it is appearing on the forehand and leg. it is getting subdued but appears again in small spots, which is a cause for concern.

I think you have taken medications for around six to seven weeks so you can continue them for another two to three weeks. Yes, if there are only two to three patches you can apply only creams it will subside you can change cream to use Terbinafine cream instead of Lulibet cream.
