Hi doctor,

I had been treated for anxiety disorder before ten years. During that period, I used to get panic attacks and skin irritation, loss of sleep, acidity, etc. I remember the doctor also prescribed me the tablet Fluoxetine that might be given to treat skin irritation. Presently, I got a habit of smoking last year, and this might be the reason I am facing skin irritation in the penile area, sometimes on my legs and hands. I am having this issue for more than a year. I took some homeopathic medicine (Natmur), followed by allopathic medicine, but in vain. Sometimes I could not sleep well, get indigestion, headache, etc. Is it possible that it is an anxiety disorder? If I have to take Fluoxetine again, will it have any side effects and that I get habitual to it? Which cream shall be used to reduce the irritation? I use to take Flunil and Clotrin also. Kindly give your opinion.

Are you diabetic? Is there any night aggravation of itching? Are web spaces of fingers involved? Do you have mite allergies? History of Atopy? Difficulty in breathing? Do you feel restless? Are you hypertensive? Do you have thyroid dermatopathy? Is there any widespread rash? Any dermatological illness? Itchy genitals? Did you have unprotected sex in the last three months? Do you have burning micturition? Is there an increased frequency of urination? Please answer the questions so that I can guide you accordingly.

Hi doctor,

Thank you. I am not diabetic. There is night aggravation of itching. Web spaces of fingers are not involved. I do not know whether I have mite allergies. There is no history of atopy. I do not have difficulty in breathing. Sometimes I feel restless. I am not hypertensive. I do not have thyroid dermatopathy. There is widespread rash. Skin irritation is there at present. Yes, too much of itchy genitals. No unprotected sex in the last three months. I do not have burning micturition. There is an increase in the frequency of urination. Kindly help me.

Scabies could be a possible diagnosis as night aggravation of itching is a typical sign of scabies. Any similar complaints in family members? Scabies is a contagious infection caused by mite Sarcoptes scabiei, which burrows under the host skin, causing an intense itching sensation due to allergic response. It is excruciatingly contagious. Dark streak surmounted at the end by a papule. Intense itching, marked at night, is an important component in arriving at a diagnosis of scabies. I suggest the following. 5 percent Permethrin (Permite) cream at night on day one and day seven (apply cream below neck all over including trunk, torso or extremities, limbs). Tablet Atarax (Hydroxyzine Hydrochloride) 10 mg thrice daily after food for seven days. Permite BB bar soap for the bath for one month. Maintain proper hygiene. Wash clothes separately in hot water and wear ironed clothes. Avoid sharing towels, soaps, belongings with closed contacts. Any closed contact affected? I hope this was helpful.
