Hi doctor,

I am a 68-year-old, with a height of 5 feet and 8 inches, and weight of 176 pounds. Two stents were placed before six years for percutaneous transluminal coronary angioplasty (PTCA) and left anterior descending artery (LAD). I am taking Ecospirin, Clopidogrel, Dytor, Zoryl, and Urimax for urine clearance. Other than these I also take Pantacid and Rosumac. I have been suffering from burning sensation in the rectum, pain, muscle collapse in the lower part of the body, and swelling on the ankle for the past four years. I took X-ray of both lumbar and cervical spine and was diagnosed with spondylosis and degeneration. Please suggest me medications.

1. Avoid lifting heavy weights.

2. Avoid bending your back, instead bend your knees to pick up things from floor.

3. Avoid sitting for long hours.

4. Avoid two wheeler rides.

5. Improve your posture while sitting.

6. Avoid sitting for more than 20 minutes. Always sit on a chair with back support.

7. Do not get up suddenly from a sleeping position. Turn towards your side and then get up.

8. Take rest if you have severe pain.

9. Give hot fomentations for the affected area twice daily.

10. I suggest (consult with a specialist doctor, talk with him or her and take medicines with their consent) if you are not allergic:

Oxalgin gel for topical application.

Tablet Pregabalin 75 mg once daily at night before food for ten days.

Tablet Paracetamol 650 mg twice daily after food for ten days.

Tablet Rabeprazole 20 mg twice daily 20 mg twice daily after food for ten days.

Tablet Shelcal 500 mg twice daily before food for 30 days.

Tablet Vitamin D (60 k units) once daily for ten days, then once a month for 11 months.

11. Avoid these tablets if you have bronchial asthma. Follow up immediately if you have any side effects.

12. Consult a physiotherapist for McKenzie back extension exercises, back core strengthening exercises, glutei strengthening exercises, and passive straight leg raising exercises and do it regularly for relief.

13. Do swimming, yoga, go for a walk or jog once the pain relieves.

14. Provide your X-ray report for diagnosis.
