Hi doctor,

I am a 21-year-old male, and I have a pain in my right testicles for the past five to six days and sometimes on my left too. I am scared of it. I cannot concentrate on my studies. A dull ache persistently occurs. I can notice some veins after this pain. I checked my testicles. Since that time, I have had pain. I did not have this problem till now. This is for the first time. The day-by-day pain is increasing. My scrotum is not as before now. Sometimes I get hurt in my stomach and sides also. Sometimes there is sudden left-side pain also. Please help.

I understand your concern. Please help me with a few details so that we can get to the bottom of your concerns: Are you diabetic or hypertensive? Have you ever had kidney stones before? Do you regularly lift heavy weights? Was this onset of pain just before any form of intense physical activity? Do you have any pain in the area between your testicles and anus? Are you sexually active, or do you masturbate regularly? Do you have any pain after ejaculation? Do you have trouble passing urine, such as straining to pass urine, increased frequency, urgency, or feeling of incomplete bladder emptying? Have you had any scans done since this pain started?

Thanks for the reply.

I do not have previous reports of diabetes or hypertension. I did not have the problem of kidney stones. But my mom and dad had a problem. I did not lift weights regularly, but occasionally, I do. As I already reported, I examined balls for testicular self-examination. From that exact time, after one hour, the pain started. I had pain between the area of the anus and testis in the first two days. More than pain, it is like cramps. I thought it was because of this summer. Then it started paining in the testicles and stomach area only. Initially, I masturbated three to four times a week. Now I reduced. I used to ejaculate significantly faster within a minute. No, I did not feel any pain after my ejaculation. No, I do not have trouble passing urine. It is normal. If I drink more water, the urine will become urgent within an hour, and I should go. Otherwise no problem with urination till now. For the past five months, I have had a problem with balanitis. I have shown it to a urologist. He said to do circumcision. But I did not do it, and I am using M-bact ointment. I have seen deviations of erection than normal, and my balls have become saggy for the past two months. I do not know the reasons for that too. Now I am facing this problem. Is this create any severe issues now or in the future ? I have noticed some veins on my testicles after this pain. I have never seen these veins before. Shall I send pictures of my testis? I did not do any scans regarding this. I am scared of it. There is slight dull pain in my right testis and also sometimes in my stomach.

I understand your concern. Please be assured that this is not any form of cancer. You most likely have prostatitis, a prostate infection commonly seen in people sitting for long hours. The other reason for the testicle pain could be the dilation of the veins, which we can pick up on ultrasound. I would advise you to get a few tests done so that we can rule everything out: Ultrasound scrotum, ultrasound abdomen, random blood sugar, and urine routine and microscopy. I would advise you to share photos of the balanitis and the testicles so I can look.
