Hi doctor,

I used a very tight condom nearly two weeks ago. It felt tight at the base during movement. I used it for about 15 minutes. Since the incident, I am having erection issues. I no longer wake up with a morning erection. For the first seven to ten days, it was painful at the base and I was unable to get an erection at all. Now, I can get an erection from a lying or sitting position, but not standing. Pain is gone. I also experienced numbness for about 10 days where my penis felt disconnected from my body. I never had trauma to the penis before. I had some erectile dysfunction and I take the tablet Tadalafil 2.5 mg daily. However, since the incident, I am having issues even with the tablet Tadalafil (I also tried a 5 mg dose). I got blood tests done around one year back and reports were fine, except for a higher cholesterol level. Is a full recovery still possible? Is there any advice to help to heal? What could this be?

I think you will need evaluation for erectile dysfunction including USG (ultrasonography) doppler to look for a venous leak. That will show us if you got an issue. Using a tight band at the base of the penis repeated times will cause damage to the blood vessels or nerves. Blood vessels might have venous leaks and nerves will cause erectile dysfunction. Treatment with the tablet Tadalafil can be 20 mg every three days. This condition might reverse to normal but needs a longer time, about months to a year.

Thanks for the response.

When lying down, I can get fine erections but when I go from lying to a standing position, the erection starts going away. If I try to initiate an erection from a standing position, the penis fills with the blood but it is like a muscle that activates an erection and is not working. I know it is difficult to say without examining me, but does this sound like something that can resolve on its own? Does it sound more like a venous leak or nerve damage to you?

Fine, so you mean that there is an erection on standing but the penis drops, or there is no erection in the standing position? In a standing position, if there is an erection but the penis drops that means your suspensory ligament is injured. And if no erection is there, it means there is a neural or venous issue.
